What if I do not see my plan listed?

Call the office. Most plans are accepted, the list changes as contracts update, and staff can verify your specific plan and benefits before your visit.

Do you accept Medicaid?

Yes. Illinois Medicaid and most Medicaid managed-care plans are accepted, along with Medicare and Medicare Advantage plans.

Do I need a referral?

PPO plans usually do not require one. HMO and many Medicaid managed-care plans do require a referral from your primary care physician before an orthopedic visit.

Do you see workers' compensation and injury cases?

Yes. All workers' compensation carriers are accepted, and third-party liability and personal injury cases are accepted on a lien basis.

Are injections covered?

Cortisone injections are typically covered. Viscosupplementation for knee arthritis is often covered with prior authorization, which the office can submit for you. PRP is not covered and is self-pay.

What should I bring to my first visit?

Your insurance card, photo ID, a referral if your plan requires one, a list of medications, and any prior imaging or reports related to the problem.
